AUSTIN, TX -- Migrant Clinicians Network (MCN) today announced that it will receive a one-year, $50,000 grant renewal from the Elton John AIDS Foundation (EJAF) to help mobile patients with HIV connect to, and stay in, health care and HIV treatment.
MCN’s Bridge to Care project, which the grant will fund, is an HIV-specific subsection of MCN’s innovative bridge case management program, Health Network [1] (HN).
“Appropriate medical care and treatment for HIV can be difficult for anyone to obtain,” said Del Garcia, MCN’s Director of International Projects, Research, and Development. “We work to make re-entry into the community easier by establishing critical linkages."
HN collaborates with providers in primary care clinics, health departments, and the federal detention system in 45 states and 70 countries to keep medically-underserved mobile patients in care. HN provides comprehensive case management, medical records transfer and follow-up services for those under care for cancer screening and treatment, diabetes and other chronic illnesses, TB and HIV, and women needing prenatal and postpartum care. Bridge to Care, part of Health Network, provides services specifically for incarcerated or detained HIV+ individuals, both pre-release to open society, and after their release in the US, or upon returning to their country of origin.
EJAF is an international non-profit organization funding direct patient care services and AIDS prevention education. This is the third consecutive year that Bridge to Care has received funding from EJAF. In addition to funding extensive case management for Bridge to Care patients to ensure continuity of care, this grant will also provide funding for education for all stakeholders, to assure optimal participation in Bridge to Care.
