Ensuring Continuity of Care for your Mobile Patients
For more than twelve years MCN has worked to address the unique challenge of mobility by creating a bridge case management system to allow clinicians follow-up with their mobile patients coupled with appropriate continuity of care planning. Health Network is comprised of 4 projects: TBNet, Diabetes Track II, the Prenatal Project and CAN-track. CAN-track is designed to increase cancer screenings for breast, cervical and colon cancers while reducing loss to follow up among mobile patients. [RSVP Information]
