Participants in the virtual learning collaborative will attend six (6) learning sessions consisteing of live web-based platform training, offline team-based activities, and facilitated office hourse.  Click here to register for the series.

PATIENT AND FAMILY ENGAGEMENT THROUGH CARE TEAMS:  This introductory session will elucidate the key elements of team-based care, including 1.  team communication, 2.  clarifying and optimizing roles, 3.  care coordination, and 4.  patient activation. This session will outline tools and strategies to engage patients as members of the care team.  Participants will be encouraged to assess their health center's level of "teamness" through an online assessment following the session.
