Clinicians serving mobile populations face unique challenges for care delivery and follow–up. Some of the challenges are high rates of ”no–shows“ and lost to follow–up and limited availability of a complete medical history. Many clinics offer outreach services and mobile clinics to be able to serve their population of focus. Nevertheless clinics are still unable to provide patients with appropriate follow–up, and lose track of them as soon as they move to a new location, especially if there is no Community Health Center in the new area. For the last eight years MCN has worked to address the unique challenge of mobility by creating a Health Network by which clinicians can follow–up their patient and patients can have appropriate continuity of care. MCN believes that mobility should not be a barrier for health care access and continuity of care.
