A Discussion of the Current Definition of Migrant Used in Migant and Community Health Centers

On many occasions, MCN staff has heard from colleagues in the area of migrant health that the official legislated definition of a Migrant or Seasonal Farmworker used in HRSA funded health centers is not representative of the population now being served in many M/CHCs. In order to have a clearer understanding of the opinions held by our colleagues, MCN initiated a 3 week-long survey in June, 2006 to gather information from individuals providing direct health services about the possible strengths or weaknesses of the definition of migrant currently in use. Here are the results.

A Guide to Integrating Community Health Workers into Health Disparity Collaboratives

The first section of the document is tailored to each Collaborative Topic (diabetes, cancer, etc.) with suggestions for how CHWs can promote significant outcomes within a variety of measures. The second section includes a grid describing roles for CHWs in five of six components of the Chronic Care Model, aligned with already-established Change Concepts such as "Set and document self-management goals collaboratively with patients." For each Change Concept, a number of high-potential Change Ideas are presented.
