The Community Health Center, Inc. (CHC) is hosting the first in a series of 8 webinars on transforming practices through the team-based model of care. This webinar will introduce our faculty from the Community Health Center, Inc. and Dr. Ed Wagner, MD, MPH and his team from the MacColl Center for Health Care Innovation at the Group Health Research Institute. This webinar will provide an introduction to the entire webinar series, offer an overview of the rationale and evidence supporting team-based care, and outline the roles and functions of the primary care team.It is part of a series of webinars offered by Community Health Center, Inc.
