Building an Asthma Care Home: Engaging Clinicians, Patients and Caregivers in Comprehensive Asthma Management

Review of The National Heart Lung and Blood Institute (NHLBI) 2007 National Asthma Education and Prevention Program Guidelines with a focus on incorporating environmental asthma trigger assessment into practice with strategies to reduce exposures to indoor air pollutants 2. Support patient and family engagement by integrating asthma education and self-management tools for patients with limited English proficiency and low health literacy 3. Understand the use of Health Information Technology to prompt adherence to national asthma guidelines, collect patient and population health data, educate patients on self-management, and facilitate quality improvement activities
