Helping Health Centers Identify Migrant and Homeless Patients

The Health Care for the Homeless (HCH) Clinicians’ Network and Migrant Clinicians Network (MCN) serve as National Partners to the HRSA Health Disparities Collaboratives. One of the critical roles for both organizations is helping health centers identify their migrant and homeless patients. Both organizations share a commitment to the principal that mobility should not be a barrier for health care access or continuity of care. What follows is a set of succinct and helpful suggestions for how to better identify homeless and migrant patients in your practice.

Clinical Services Consultation

MCN's expert clinical team can provide both on-site and distance consultation services. The experienced team provides a number of critical services including consultation on clinic systems, protocols and procedures as well as program development and evaluation; patient tracking and bridge case management; environmental and occupational medicine, pesticide awareness, and worker’s compensation issues for health centers; immigration and how it effects health centers.

Immunization Initiative

The Immunization Initiative at the Migrant Clinicians Network (MCN) is devoted to promoting and improving childhood, adolescent, and adult immunization coverage levels among migrant and other mobile underserved populations. Funding for this project is obtained from a cooperative agreement with the National Immunization Program, Centers for Disease Control and Prevention.
